⚠️ Safety Note: Oxygen supports combustion - it is not itself flammable but intensifies fire risk. Patients must stop smoking completely before starting home oxygen.
Measurements should be made with the patient on optimal medical therapy, breathing room air for ≥20 min, at rest, and in a stable clinical state.
| Aspect | Recommendation | Notes |
|---|---|---|
| Indication | PaO₂ < 7.3 kPa or 7.3–8.0 kPa with cor pulmonale / polycythaemia | Recheck when clinically stable. |
| Duration | ≥ 15 h / day (ideally during sleep and day) | Shown to improve 5‑year survival. |
| Target PaO₂ / SpO₂ | PaO₂ > 8 kPa (≈ SpO₂ ≥ 90%) | Avoid hyperoxia in CO₂ retainers. |
| Delivery | O₂ concentrator primary device | Portable units for ambulation. |
| Monitoring | ABG every 6 m, O₂ usage meter | Watch for CO₂ retention. |
| Safety | No smoking / fire proximity | Education essential. |
