• 📉 Anaemia or active haemorrhage.

• ⚠️ Reduce dose in renal impairment (contraindicated if CrCl <30 mL/min).

• Routine anti-Factor Xa activity monitoring not required.
• Consider in pregnancy, extremes of weight, or severe renal impairment.
