- ๐ Benzylpenicillin (Penicillin G) โ one of the oldest and most important beta-lactam antibiotics.
- โ ๏ธ Deactivated by beta-lactamaseโproducing bacteria.
- ๐ซ Cannot be given orally (acid-labile, destroyed by gastric acid).
โก Mechanism of Action
- ๐ช Bactericidal: inhibits bacterial cell wall synthesis.
- Prevents formation of peptidoglycan cross-links โ weakens cell wall โ lysis.
๐งฌ Spectrum of Activity
- โ
Streptococci, Pneumococci, Meningococci.
- โ
Neisseria gonorrhoeae (though resistance now common).
- โ
Treponema pallidum (syphilis โ gold standard).
- โ
Actinomyces israelii.
- โ
Anaerobes of mouth/upper GI tract.
- โ Not effective against Bacteroides fragilis (lower bowel anaerobe).
๐ฉบ Indications
- ENT & respiratory: throat infections, otitis media, pneumonia.
- Cardiac: infective endocarditis.
- CNS: meningitis, neurosyphilis, brain abscess.
- Genitourinary: syphilis, gonorrhoea.
- Other: actinomycosis, anthrax, gas gangrene, diphtheria, leptospirosis, Lyme disease.

๐ Interactions
- โฌ๏ธ Probenecid reduces renal excretion โ raises serum levels (sometimes used therapeutically).
- โฌ๏ธ Risk of methotrexate toxicity (reduced clearance).
โ ๏ธ Cautions
- ๐จ Allergy โ risk of hypersensitivity/anaphylaxis.
- ๐งช May cause false-positive urine dipstick for glucose.
- ๐ Adjust dose in renal impairment.
๐ซ Contraindications
- Known penicillin allergy (consider erythromycin or doxycycline as alternatives).
๐ฅ Side Effects
- ๐ซ Allergic reactions: rash, urticaria, fever, angioedema, anaphylaxis.
- ๐ฉบ Renal: interstitial nephritis.
- ๐ฉธ Haematological: haemolytic anaemia, leucopenia, thrombocytopenia.
- ๐ฉ GI: antibiotic-associated colitis (rare, but important).
- ๐ง High IV doses: neurotoxicity, convulsions.
