Creatinine Clearance
📑 Summary
⚠️ The Cockcroft–Gault formula assumes normal muscle mass. It may be inaccurate in muscle wasting, neuromuscular disease, or extremes of body habitus.

- 🧮 Widely used for >40 years to estimate renal function.
- ⚠️ Inaccurate with extremes of age, weight, or muscle mass.
- 💊 Creatinine clearance (CrCl) is often preferred over eGFR for drug dosing.
🧪 Physiology
- Creatinine = byproduct of muscle metabolism, cleared by kidneys.
- 🩸 Minimal reabsorption → a good marker of renal clearance.
- CrCl ≈ GFR (adjusted by age, sex, body size).
- Normal values in healthy young adults:
- 👩 Women ≈ 95 mL/min
- 👨 Men ≈ 120 mL/min

- Needs: 👤 Sex | 🎂 Age | ⚖️ Weight | 🧪 Serum creatinine (± Height for BSA).
- Formula:
CrCl (mL/min) = ((140 - Age) × Weight (kg) × [constant]) / Serum Creatinine (mg/dL)- Constant = 1.23 (men), 1.04 (women)
- ⚠️ Use with caution in elderly or those with abnormal muscle mass.
