- โฌ๏ธ Sebum production.
- โฌ๏ธ Keratinisation and follicular occlusion.
- Indirect antimicrobial effect (โ Propionibacterium acnes growth).
๐ Indications
- Severe acne vulgaris resistant to conventional treatment.
- Papulopustular rosacea (specialist use).
- Occasionally severe seborrhoea or hidradenitis suppurativa (off-label).
๐ Interactions
- โ ๏ธ Avoid with Vitamin A โ risk of hypervitaminosis A (stomatitis, epistaxis, skin changes).
- โ ๏ธ Avoid with tetracyclines โ risk of idiopathic intracranial hypertension.
โ ๏ธ Cautions
- Fertile females โ strict pregnancy prevention programme.
- Psychiatric history (depression, suicidal ideation) โ monitor mood closely.
- Regular monitoring of LFTs and lipids (triglycerides may rise).
โ Contraindications
- ๐ซ Pregnancy or breastfeeding.
- ๐ซ Sexually active fertile females not using effective contraception.
- ๐ซ Renal or hepatic impairment.
- ๐ซ Hypertriglyceridaemia.
- ๐ซ Hypervitaminosis A.
๐ฅ Side Effects
- ๐ง Dry mucous membranes & skin, cheilitis, epistaxis, alopecia.
- ๐งช โ Triglycerides, hepatitis, raised LFTs.
- ๐ง Idiopathic intracranial hypertension (rare, esp. with tetracyclines).
- ๐ง Mood changes, depression, rarely psychosis/suicidality.
- ๐ฉธ Anaemia, thrombocytopenia, neutropenia (rare).
๐ Dose
- 0.5โ1 mg/kg/day PO for 16โ24 weeks (โ4โ5 months).
- Doses up to 2 mg/kg/day may be used for severe or truncal acne.
- Repeat course may be considered if relapse occurs after at least 8 weeks off treatment.
