- ๐ First-generation sedating antihistamine.
- Widely used in allergy and emergency settings (e.g. anaphylaxis protocols).
โ๏ธ Action
- Antagonist at Histamine H1 receptors.
- Blocks the effects of histamine released from mast cells and basophils.
- Has additional mild antimuscarinic effects (explains side effects).
๐ Indications / Dose
- ๐คง Allergy (hay fever, urticaria, insect bites): 4 mg PO every 4โ6 hours (max 24 mg/day).
- ๐ Anaphylaxis / acute allergy (adjunct to adrenaline): 10 mg slow IV injection over 1 minute (can also be given IM).
- Paediatric dosing differs โ check BNF carefully.
๐ Interactions
- ๐ท Alcohol โ increases sedation.
- ๐ MAOIs & TCAs โ increase antimuscarinic effects.
- โฌ๏ธ May increase phenytoin plasma levels.
โ ๏ธ Cautions
- ๐ด BPH / prostatism โ risk of urinary retention.
- ๐๏ธ Glaucoma.
- โก Epilepsy, severe hypertension, cardiovascular disease.
- ๐ Sedation โ caution with driving or operating machinery.
โ Contraindications
- ๐ซ Known hypersensitivity to chlorphenamine or excipients.
๐ฅ Side Effects
- ๐ด Sedation, drowsiness, fatigue (less than older antihistamines, but still significant).
- ๐คข Antimuscarinic effects: dry mouth, blurred vision, constipation, urinary retention, nausea, dizziness.
- โค๏ธ Palpitations, tachycardia.
- ๐ง Rare: paradoxical CNS stimulation (especially in children, can cause irritability or insomnia).
- ๐ฉบ Rare: liver dysfunction, allergic skin reactions.
