| Definition |
Inflammation of the labyrinth (inner ear) affecting both hearing and balance. |
Inflammation of the vestibular nerve affecting balance only. |
| Common Causes |
- ๐ฆ Viral (e.g. HSV, influenza, adenovirus)
- ๐งซ Bacterial (from otitis media or meningitis)
- ๐ค Trauma, allergy, ototoxic drugs (rare)

| Otoscopy / Hearing Tests |
- Normal tympanic membrane unless concurrent otitis media
- Sensorineural hearing loss on tuning fork or audiometry
- Positive Rinne and Weber lateralises to good ear

| Diagnosis |
- Clinical diagnosis (vertigo + hearing loss)
- Audiometry: confirms SNHL
- MRI brain/IAC if atypical features (exclude stroke or acoustic neuroma)

- Head impulse test positive (peripheral vestibular lesion)
- MRI if central signs or persistent symptoms

| Treatment |
- ๐ฏ Treat cause: Antivirals or antibiotics if indicated
- ๐ Symptomatic:
โ Prochlorperazine or Cyclizine (โค3 days)
โ Ondansetron if severe vomiting
- ๐ Short course of oral corticosteroids (may aid recovery)
- ๐ถโโ๏ธ Vestibular rehabilitation therapy (VRT)
- ๐ง Hearing may recover over weeks; counsel about safety (e.g. driving).

- ๐ Corticosteroids (early use may improve recovery)
- ๐ฏ Symptomatic relief as above
- ๐ถโโ๏ธ VRT: critical for neuroplastic compensation
- โ Avoid prolonged vestibular suppressants - delay recovery

| Prognosis |
- โณ Recovery in 2โ6 weeks; hearing often improves but may remain reduced.
- ๐งญ Some develop chronic imbalance or anxiety-related dizziness.

- โณ Usually resolves fully in weeks; recurrent episodes are rare.
- ๐ง Brain compensation restores balance via central adaptation.
