Causes of Galactorrhoea

Galactorrhoea = inappropriate, non-puerperal milk secretion.
Most commonly due to hyperprolactinaemia, but always first exclude pregnancy and review the drug history.
๐ Causes
- ๐ง Prolactinoma (pituitary adenoma): Galactorrhoea, amenorrhoea, infertility, headaches, visual field loss.
- ๐ฆ Hypothyroidism: TRH stimulates prolactin โ galactorrhoea with fatigue, weight gain, cold intolerance.
- ๐ Medications: Antipsychotics, SSRIs, TCAs, methyldopa, verapamil.
- ๐ฉธ Chronic kidney disease: Reduced clearance of prolactin โ galactorrhoea with CKD features (fatigue, oedema).
- โ Idiopathic: Diagnosis of exclusion; normal cycles, mild prolactin rise or normal.

- ๐ Serum prolactin โ persistently elevated suggests prolactinoma or secondary cause.
- ๐ฆ Thyroid function tests โ exclude hypothyroidism.

- ๐ง MRI pituitary if prolactin persistently high.
- ๐ Visual fields if pituitary adenoma suspected.
๐ Management
- ๐ง Prolactinoma: Dopamine agonists (cabergoline, bromocriptine). Trans-sphenoidal surgery if resistant/large. Radiotherapy rarely.
- ๐ฆ Hypothyroidism: Levothyroxine replacement โ prolactin normalises.
- ๐ Drug-induced: Stop/switch offending drug if possible; dopamine agonists if severe symptoms and cannot stop.
- ๐ฉธ CKD: Optimise CKD (dialysis, transplant). Dopamine agonists for symptomatic relief.
- โ Idiopathic: Observation if mild. Dopamine agonists if persistent/bothersome.
