📚 NICE Guidance for Medical Students - Part 3 This section adds more high-yield topics across neurology, gastroenterology, renal, respiratory, dermatology, rheumatology, ENT, ophthalmology, haematology and prescribing safety. For MLA/finals, learn the pattern, red flags, first-line action and referral threshold.

🧠 Epilepsy - NICE NG217

• Do not diagnose epilepsy from one collapse alone - take a careful witness history.
• Ask about aura, tonic-clonic movements, tongue biting, incontinence, post-ictal confusion, triggers and recovery time.
• Syncope mimics seizure: prodrome, pallor, sweating, brief LOC and rapid recovery suggest syncope.
• After a first suspected seizure: check glucose, ECG, U&E, calcium, magnesium and consider neuroimaging/EEG via specialist pathway.
• Status epilepticus is an emergency: ABCDE, glucose, benzodiazepine, second-line antiseizure medication and anaesthetic/ICU escalation if refractory.
• Epilepsy has important safety counselling: driving, swimming, heights, machinery, baths, pregnancy and contraception interactions.

🧠 Headache Red Flags

• Thunderclap headache - subarachnoid haemorrhage until proven otherwise.
• New headache with focal neurology, seizure, papilloedema or reduced consciousness.
• Headache with fever, neck stiffness or rash - meningitis/encephalitis.
• New headache after age 50 with scalp tenderness/jaw claudication - giant cell arteritis.
• Progressive morning headache with vomiting - raised intracranial pressure.
• Headache in pregnancy/postpartum - consider pre-eclampsia, venous sinus thrombosis, pituitary apoplexy.
• Medication-overuse headache: frequent analgesic/triptan use can perpetuate chronic headache.

🧠 Parkinson’s Disease

• Classic triad: bradykinesia, rigidity and resting tremor.
• Red flags for atypical parkinsonism: early falls, early autonomic failure, vertical gaze palsy, poor levodopa response, rapid progression.
• Do not stop Parkinson’s medicines abruptly - risk of severe rigidity, aspiration, delirium and neuroleptic malignant-like syndrome.
• Avoid dopamine-blocking antiemetics/antipsychotics such as metoclopramide, prochlorperazine and haloperidol where possible.
• Safer nausea options often include domperidone with QT/cardiac caution, or specialist advice.
• Falls, dysphagia, constipation, hallucinations, orthostatic hypotension and cognition are core complications.

🧠 Multiple Sclerosis

• Suspect with episodes of neurological symptoms separated in time and anatomical location.
• Common features: optic neuritis, sensory symptoms, limb weakness, diplopia, ataxia, bladder dysfunction and fatigue.
• MRI brain/spine supports diagnosis; lumbar puncture may show oligoclonal bands.
• A relapse is new or worsening neurological symptoms lasting more than 24 hours without infection/fever.
• Exclude pseudo-relapse triggers such as UTI, fever and heat before steroids.
• High-dose corticosteroids can shorten relapse duration but do not change long-term disability.

🧠 Migraine

• Migraine is usually episodic, unilateral or throbbing, associated with nausea, photophobia or phonophobia.
• Aura is a reversible neurological symptom, often visual, developing gradually.
• Acute treatment often uses NSAID/paracetamol ± triptan ± antiemetic depending on patient factors.
• Consider preventive therapy if frequent, disabling or medication overuse risk.
• Avoid combined hormonal contraception in migraine with aura due to stroke risk.
• Red flags always override migraine label.

🫁 Stable Asthma Reviews

• Ask about daytime symptoms, night waking, reliever use, activity limitation and exacerbations.
• Check inhaler technique and adherence before stepping up treatment.
• Personalised asthma action plans reduce risk.
• Excess SABA use is a danger signal.
• Consider occupational triggers, smoking/vaping, rhinitis, reflux, obesity and poor technique.
• Acute deterioration: assess PEF, SpO₂, ability to speak, respiratory rate, pulse and exhaustion.

🫁 COPD Reviews

• Confirm diagnosis with post-bronchodilator spirometry showing persistent airflow obstruction.
• Assess exacerbation frequency, breathlessness, smoking, inhaler technique, oxygen saturations, weight and exercise tolerance.
• Smoking cessation is the most disease-modifying intervention.
• Offer pulmonary rehabilitation for function-limiting breathlessness where appropriate.
• Long-term oxygen therapy is for chronic hypoxaemia, not just breathlessness.
• Consider heart failure, anaemia, lung cancer, bronchiectasis and PE if symptoms change unexpectedly.

🦠 Tuberculosis

• Think TB with chronic cough, weight loss, night sweats, fever, haemoptysis or lymphadenopathy.
• Risk factors: close contact, homelessness, prison, immunosuppression, high-incidence country exposure.
• Pulmonary TB needs sputum AFB smear/culture and NAAT where available.
• TB treatment requires multidrug therapy and public health notification.
• Do not start steroids or biologics without considering latent TB risk in relevant patients.
• Spinal TB can present with back pain and neurological compromise.

🍽 Dyspepsia / GORD / Peptic Ulcer Disease

• Red flags: dysphagia, weight loss, persistent vomiting, GI bleeding, iron-deficiency anaemia, epigastric mass.
• Review NSAIDs, aspirin, steroids, anticoagulants, smoking and alcohol.
• Test and treat H. pylori where appropriate.
• PPI trial is common for reflux/dyspepsia without alarm features.
• Upper GI bleed: resuscitate first; endoscopy after stabilisation.
• Perforation: sudden severe pain, peritonism, free air - urgent surgery.

🧬 Inflammatory Bowel Disease

• UC: bloody diarrhoea, urgency, tenesmus, continuous colonic inflammation from rectum proximally.
• Crohn’s: abdominal pain, weight loss, diarrhoea, perianal disease, skip lesions, small bowel involvement.
• Faecal calprotectin helps distinguish inflammatory from functional bowel symptoms in many settings.
• Acute severe colitis: frequent bloody stools plus systemic toxicity - admit under gastroenterology.
• Avoid loperamide/opioids in severe colitis due to toxic megacolon risk.
• IBD patients may have extraintestinal features: arthritis, erythema nodosum, uveitis, PSC.

🍞 Coeliac Disease

• Features: diarrhoea, bloating, weight loss, fatigue, iron-deficiency anaemia, mouth ulcers, infertility or osteoporosis.
• Check total IgA and tissue transglutaminase IgA while the patient is still eating gluten.
• Do not start gluten-free diet before testing unless specialist-directed, because it can make tests falsely negative.
• Associated with type 1 diabetes, autoimmune thyroid disease and dermatitis herpetiformis.
• Treatment is lifelong gluten-free diet with dietitian support.
• Monitor nutritional deficiencies, bone health and symptom response.

🫘 Chronic Kidney Disease - NICE NG203

• CKD is defined by reduced eGFR and/or markers of kidney damage persisting for at least 3 months.
• Use both eGFR and urine ACR - albuminuria is a major risk marker.
• Common causes: diabetes, hypertension, glomerulonephritis, reflux/obstruction, polycystic kidney disease.
• Manage BP, diabetes, cardiovascular risk, proteinuria and nephrotoxic medicines.
• ACEi/ARB is useful for albuminuria but monitor creatinine and potassium after starting/increasing.
• Refer if rapid eGFR decline, heavy proteinuria, haematuria with proteinuria, resistant hypertension, suspected renal artery stenosis or genetic/rare disease.

🧪 Nephrotic / Nephritic Patterns

• Nephrotic: heavy proteinuria, hypoalbuminaemia, oedema and hyperlipidaemia.
• Nephritic: haematuria, proteinuria, hypertension, AKI and red-cell casts.
• Nephrotic syndrome increases VTE and infection risk.
• Nephritic syndrome can progress rapidly and needs urgent nephrology if AKI, hypertension or systemic features.
• Always ask about rash, arthritis, sinus/lung symptoms, haemoptysis, drugs and infections.

🦴 Rheumatoid Arthritis - NICE NG100

• Suspect inflammatory arthritis with persistent synovitis, morning stiffness, swollen MCP/PIP/wrist joints and symmetrical small-joint involvement.
• Refer early to rheumatology if persistent synovitis, even if RF/anti-CCP are negative.
• DMARDs should be started early under specialist care to prevent erosions and disability.
• Steroids may bridge symptoms but are not long-term disease control.
• Monitor infection risk, vaccinations, blood tests and drug toxicity on DMARDs.
• RA is systemic: consider anaemia, ILD, cardiovascular risk, nodules, scleritis and cervical spine disease.

🦵 Osteoarthritis - NICE NG226

• Diagnosis is usually clinical in adults over 45 with activity-related joint pain and no prolonged morning stiffness.
• Core treatments: education, exercise, weight management where appropriate, and self-management.
• Exercise helps pain and function even when joints hurt initially.
• Topical NSAIDs are often preferred before oral NSAIDs, especially in older patients.
• Do not rely on X-ray severity alone - symptoms and function matter.
• Refer for joint replacement when symptoms substantially affect quality of life and non-surgical options are inadequate.

🧬 Gout

• Acute hot swollen joint, classically first MTP, but ankle/knee/wrist can occur.
• Diagnose by crystal confirmation if uncertain or atypical.
• Acute treatment: NSAID, colchicine or corticosteroid depending on comorbidity.
• Do not start urate-lowering therapy during acute attack unless already planned/specialist advice; if already taking allopurinol, usually continue it.
• Long-term urate lowering is treat-to-target and requires flare prophylaxis when starting.
• Consider CKD, diuretics, alcohol, obesity and metabolic syndrome.

🦋 SLE / Vasculitis Clues

• SLE: photosensitive rash, oral ulcers, arthritis, serositis, cytopenias, renal disease, neuropsychiatric symptoms.
• Vasculitis: systemic symptoms plus organ-specific ischaemia/inflammation - rash, neuropathy, renal disease, pulmonary haemorrhage, ENT symptoms.
• Red flags: haematuria/proteinuria, AKI, haemoptysis, mononeuritis multiplex, purpura, visual symptoms.
• Check urine dip in suspected systemic autoimmune disease.
• Urgent rheumatology/nephrology if organ-threatening disease.

🩹 Eczema / Psoriasis / Cellulitis Distinctions

• Eczema: itch, flexural pattern, dry inflamed skin, atopy.
• Psoriasis: well-demarcated plaques with silvery scale, extensor surfaces, scalp/nails.
• Cellulitis: unilateral warmth, erythema, pain, swelling; systemic features increase concern.
• Bilateral red legs are often venous eczema/lipodermatosclerosis rather than bilateral cellulitis.
• Do not give repeated antibiotics for inflammatory skin disease without infection features.
• Urgent concern: rapidly progressive pain out of proportion = necrotising fasciitis until proven otherwise.

🧴 Acne / Rosacea

• Acne severity depends on comedones, inflammatory lesions, nodules, scarring and psychological impact.
• Topical retinoids/benzoyl peroxide/antibiotic combinations are common first-line options depending on severity.
• Avoid antibiotic monotherapy; combine with non-antibiotic topical treatment to reduce resistance.
• Isotretinoin is specialist and teratogenic - pregnancy prevention is essential.
• Rosacea: flushing, erythema, papules/pustules, telangiectasia; avoid steroid creams on face unless specifically indicated.

👁 Red Eye

• Benign conjunctivitis usually has irritation/discharge but normal vision and no severe pain.
• Red flags: reduced vision, severe pain, photophobia, contact lens use, corneal opacity, abnormal pupil, trauma, chemical injury.
• Contact lens red eye = microbial keratitis until proven otherwise.
• Acute angle closure: painful red eye, haloes, headache/vomiting, fixed mid-dilated pupil.
• Anterior uveitis: photophobia, ciliary flush, small/irregular pupil.
• Chemical injury: irrigate immediately before referral.

👂 Hearing Loss / Vertigo

• Conductive hearing loss: wax, otitis media, perforation, otosclerosis; Rinne negative, Weber to affected ear.
• Sensorineural hearing loss: presbycusis, noise, Ménière’s, vestibular schwannoma; Weber to better ear.
• Sudden sensorineural hearing loss is an ENT emergency.
• Vertigo with focal neurology, new severe headache, ataxia or inability to walk = central cause until proven otherwise.
• BPPV: brief positional vertigo; vestibular neuritis: prolonged vertigo without hearing loss; labyrinthitis includes hearing symptoms.

👃 ENT Red Flags

• Stridor, drooling, inability to swallow saliva or respiratory distress.
• Quinsy: hot-potato voice, trismus, unilateral tonsillar swelling and uvular deviation.
• Epiglottitis: severe sore throat, drooling, muffled voice, stridor - do not distress the patient.
• Septal haematoma after nasal trauma needs urgent drainage.
• Malignant otitis externa: severe ear pain in diabetes/immunosuppression with granulation tissue.
• Head and neck cancer red flags: persistent hoarseness, neck lump, dysphagia, unexplained ulcer, weight loss.

🩸 Haematology Patterns

• Iron deficiency: microcytosis, low ferritin, consider GI/gynae blood loss.
• B12 deficiency: macrocytosis, neuropathy, glossitis; treat promptly if neurological features.
• Haemolysis: anaemia, jaundice, high LDH/bilirubin, low haptoglobin, reticulocytosis.
• Myeloma: bone pain, anaemia, renal impairment, hypercalcaemia, recurrent infection.
• Leukaemia: fatigue, infections, bruising/bleeding, blasts, bone pain, lymphadenopathy.
• Neutropenic sepsis: fever after chemotherapy = IV antibiotics urgently.

🩸 VTE and Anticoagulation

• DVT: unilateral calf swelling/pain; PE: pleuritic chest pain, dyspnoea, syncope, haemoptysis.
• Use Wells-style probability assessment and D-dimer only in appropriate low-risk contexts.
• Anticoagulation choice depends on renal function, weight, cancer, pregnancy, bleeding risk and interactions.
• Pregnancy-associated VTE usually uses LMWH, not DOACs.
• Major bleeding on anticoagulation needs resuscitation, source control, reversal consideration and senior/haematology advice.

💊 Prescribing: Renal Dose and Sick-Day Thinking

• Check renal function before prescribing DOACs, metformin, opioids, antibiotics, gabapentinoids, digoxin and NSAIDs.
• AKI risk drugs: NSAIDs, ACEi/ARB, diuretics, aminoglycosides, contrast, trimethoprim.
• In dehydration/vomiting/sepsis, some medicines may need temporary withholding depending on clinical context.
• Metformin: consider withholding in severe AKI, hypoxia, sepsis or lactic acidosis risk.
• NSAIDs worsen renal perfusion and can precipitate AKI, GI bleeding and heart failure.

💊 Opioid Safety

• Check opioid naïve vs tolerant status.
• Common harms: respiratory depression, sedation, constipation, falls, delirium, nausea and urinary retention.
• Always prescribe laxatives when starting regular opioids unless contraindicated.
• Renal impairment increases toxicity risk, especially with morphine metabolites.
• Naloxone reverses opioid toxicity but may need repeated dosing/infusion for long-acting opioids.

💊 Steroid Safety

• Long-term steroids increase risk of adrenal suppression, osteoporosis, diabetes, hypertension, infection, mood change and skin thinning.
• Do not stop long-term steroids abruptly.
• Consider bone protection, gastric risk, glucose monitoring and infection risk.
• Patients with adrenal suppression may need stress-dose steroids during severe illness/surgery.
• Adrenal crisis: hydrocortisone immediately, then fluids and trigger management.

🧠 Teaching Note

The easiest way to learn NICE-style medicine is to build recognition maps. For each symptom, ask: what is common, what is dangerous, and what single action must happen now? For example, headache is often migraine, but thunderclap headache means SAH; red eye is often conjunctivitis, but pain with reduced vision means urgent ophthalmology; breathlessness is often COPD/asthma, but hypoxia with pleuritic pain means PE must be considered. This pattern-based approach is exactly how finals and MLA questions test safe clinical judgement.
