Octreotide is a synthetic analogue of somatostatin, used in the management of variceal bleeding ๐ฉธ, acromegaly ๐ง , and neuroendocrine tumours ๐ฏ.
โ๏ธ Mode of Action
- ๐ Mimics endogenous somatostatin but with a longer half-life.
- ๐ฉธ Reduces splanchnic blood flow โ useful in variceal haemorrhage.
- ๐งช Inhibits secretion of several hormones (GH, insulin, glucagon, vasoactive peptides).
- ๐ฝ๏ธ Suppresses GI motility and pancreatic secretions.
๐ Indications / Dose
- Variceal Upper GI Bleed ๐ฉธ: 100 mcg IV bolus stat, then infusion 25โ50 mcg/hour for 3โ5 days.
- Acromegaly ๐ง : SC injections 50โ100 mcg TDS (titrated as needed).
- Neuroendocrine tumours ๐ฏ (e.g. carcinoid syndrome): Symptom control (flushing, diarrhoea).
- VIPoma/Glucagonoma ๐ฌ: Reduce diarrhoea & electrolyte disturbances.

- โ๏ธ Alters glucose control โ monitor if on antidiabetic drugs.

- ๐ชจ Risk of gallstones with long-term use.
- ๐ญ Diabetes: risk of hypo- or hyperglycaemia.
- ๐งพ Monitor in renal/hepatic impairment.

๐ฌ Side Effects
- ๐คข GI: Nausea, vomiting, cramps, diarrhoea/constipation.
- ๐ญ Metabolic: Hyper/hypoglycaemia.
- โค๏ธ Cardiac: Bradycardia, altered heart rate.
- ๐ธ Skin: Rash, pruritus, injection site pain.
- ๐ค Neurological: Headache, dizziness.
- ๐ชจ Long-term: Gallstones (cholelithiasis).
