- ๐ฉบ Benign ovarian cysts can cause acute abdominal pain, particularly when complications arise.
๐งฌ Aetiology
- ๐ Dermoid cysts: These cysts have a long pedicle, which can twist (torsion) and cause severe pain.
- ๐ฅ Rupture or bleeding: This can occur suddenly and lead to severe pain or haemorrhage.
- ๐ฉธ Infarction: The cyst can lose its blood supply, causing infarction and intense pain.
๐ฉน Clinical Features
- โก Acute, severe abdominal pain, often in the lower abdomen.
- ๐คฒ Signs of tenderness, guarding, and lower abdominal peritonitis may be present.
- ๐ Cramp-like pain that can be intense and persistent.
๐งช Investigations
- ๐งพ Blood tests: Full blood count (FBC), urea and electrolytes (U&E), C-reactive protein (CRP).
- ๐งฌ Beta hCG: To exclude pregnancy as a differential diagnosis.
- ๐ฅ๏ธ Imaging: CT scan of the abdomen may be indicated for further assessment.
๐ Management
- ๐ฌ Laparoscopy: This may be required for fixation, ligation, and removal of the cyst, particularly if torsion or rupture is suspected.
- ๐ซ Avoid rupture: Care must be taken to prevent rupture of the cyst, especially if there is a concern about malignancy.
- ๐ฉโโ๏ธ Always involve the gynaecology team for further management and surgical intervention as needed.
