โ ๏ธ About 1โ6% of penicillin-sensitive patients will also be allergic to cephalosporins.
โ Those with a history of immediate hypersensitivity to penicillin or other beta-lactams should not receive cephalosporins.
โ ๏ธ Cephalosporins should be used with caution in patients with any beta-lactam sensitivity.

- ๐ 3rd Generation cephalosporin.
- ๐ฆ Excellent activity against Gram-negative bacilli.
- โฌ๏ธ Reduced Gram-positive cover compared with 2nd generation cephalosporins.
โ๏ธ Action
- ๐ Inhibits peptidoglycan cross-linking โ prevents bacterial cell wall formation.
- ๐ฆ Bactericidal activity with good CSF penetration (useful in meningitis).
๐ Indications
- ๐ฅ Bacterial meningitis, septicaemia, pneumonia.
- ๐งฌ Gonorrhoea, biliary sepsis, peritonitis.
- ๐ง UTIs and pyelonephritis.
- ๐ฆ Uncomplicated gonorrhoea: 500 mg IM.
- โ๏ธ Mildโmoderate infection: 1 g IV/IM BD.
- ๐ฅ Severe infection: 2 g IV QDS (max 12 g/day).
- ๐ Suspected meningitis pre-hospital (if IV benzylpenicillin unavailable): 1 g IV/IM.

๐ Interactions
- โ ๏ธ May increase effect of warfarin โ monitor INR closely.
โ ๏ธ Cautions
- ๐ซ Penicillin or cephalosporin allergy.
- ๐ง Renal impairment (dose adjustment needed).
- ๐งช False positives: urine glucose dipstick, Coombsโ test.
- ๐ IV doses may be high in sodium or potassium.
โ Contraindications
- Severe hypersensitivity reactions to beta-lactams.
- Penicillin allergy (cross-sensitivity ~10%).
๐ฅ Side Effects
- ๐คข GI: diarrhoea, antibiotic-associated colitis, nausea, vomiting.
- ๐ก๏ธ Allergic: rash, urticaria, StevensโJohnson syndrome, TEN, anaphylaxis.
- ๐งช Hepatic: hepatitis, cholestatic jaundice.
- ๐ฉธ Haematological: โ WCC, platelets, granulocytes, aplastic anaemia.
- ๐ง Renal: interstitial nephritis.
- ๐ง CNS: headache, delirium.
- โก Cardiac: arrhythmias with rapid IV injection.
