โ๏ธ Mode of Action
- Centrally acting non-opioid analgesic.
- Inhibits reuptake of serotonin, norepinephrine, and dopamine โ enhances descending inhibitory pain pathways.
๐ Indications & Dose
- Moderate pain (alternative when opioids unsuitable):
- Initial: 30โ60 mg TDS.
- Elderly: Start with 30 mg TDS to reduce risk of side effects.
- Maintenance: 30โ90 mg TDS.
๐ Interactions & Cautions
- Enhanced sedation with alcohol, opioids, sedatives, or hypnotics.
- Caution in patients with history of urinary retention or angle-closure glaucoma.
- Can cause tachycardia โ caution in cardiovascular disease.
๐ซ Contraindications
- Convulsive disorders (e.g. epilepsy).
- Acute coronary syndromes (risk of tachycardia/arrhythmias).
โ Side Effects
- Common: Dry mouth, dizziness, nausea, urinary retention.
- Less common: Blurred vision, confusion, drowsiness, headache, hallucinations.
