Tinzaparin (Innohep)

- 💉 Tinzaparin – a Low Molecular Weight Heparin (LMWH).
- 🎯 More predictable anticoagulant effect than unfractionated heparin (UFH).
⚡ Mechanism of Action
- 🔗 Binds to antithrombin → accelerates inactivation of Factor Xa.
- ⬇️ Reduces thrombin generation and clot formation.
- 📊 Predictable dose–response allows once-daily dosing without routine monitoring (except in special groups).

⚠️ Interactions
- See LMWH section – ↑ bleeding risk with other anticoagulants, antiplatelets, fibrinolytics.
⚠️ Cautions
- ⬇️ Reduce dose in renal failure.
- 📊 Monitor anti-Factor Xa in: renal impairment, pregnancy, liver failure, or extremes of body weight.
- 🩸 Increased bleeding risk with invasive procedures or trauma.
🚫 Contraindications
- Same as other LMWHs (see general LMWH guidance).
- ❌ Severe renal failure (CrCl <30 mL/min).
- Active major bleeding or history of HIT.
💥 Side Effects
- 🩸 Bleeding (dose-related).
- ⬇️ Thrombocytopenia (HIT risk – usually 5–10 days after start).
- ⬆️ Raised AST/ALT (transient).
- 🦴 Long-term use: osteoporosis (rare, with extended therapy).
