โ๏ธ Mode of Action
- Solifenacin is a selective muscarinic receptor antagonist.
- Acts mainly on the bladder detrusor muscle โ โ involuntary contractions, โ bladder capacity, โ urgency/frequency.
๐ Indications / Dose
- ๐ง Overactive bladder (frequency, urgency, incontinence): 5 mg once daily; may โ to 10 mg once daily if tolerated.
๐ Interactions
- Metabolised by CYP3A4 โ caution with potent inhibitors (e.g. ketoconazole, clarithromycin, ritonavir).
- โ ๏ธ Concomitant use with other antimuscarinics โ additive side effects (dry mouth, constipation, urinary retention).
โ ๏ธ Cautions
- ๐ QT prolongation: avoid in patients with known QT prolongation or on other QT-prolonging drugs.
- ๐งช Dose reduction required in significant renal or hepatic impairment.
- ๐ต Elderly: โ susceptibility to CNS effects (confusion, drowsiness).
โ Contraindications
- ๐ง Urinary retention.
- ๐ฝ๏ธ Severe GI disease (toxic megacolon, severe UC, GI obstruction).
- ๐ช Myasthenia gravis.
- ๐๏ธ Uncontrolled narrow-angle glaucoma.
- Always confirm via BNF.
๐ค Side Effects
- Common: ๐ Dry mouth, ๐ฝ Constipation, ๐ Blurred vision, ๐คข Dyspepsia/nausea.
- Others: Acid reflux, peripheral oedema, urinary retention.
- CNS (esp. elderly): Drowsiness, dizziness, confusion.
