- ๐ Potent vasodilator, originally developed for severe hypertension.
- ๐งด Topical formulations promote hair growth (male & female pattern baldness).
โ๏ธ Mode of Action
- Opens ATP-sensitive potassium channels in vascular smooth muscle โ hyperpolarisation โ relaxation.
- ๐ฉธ Causes marked arteriolar vasodilatation โ โ systemic vascular resistance & BP.
- Compensatory โ heart rate & cardiac output (sympathetic activation).
๐ Indications
- ๐ซ Severe or resistant hypertension (usually with a diuretic & ฮฒ-blocker to counter side effects).
- ๐โโ๏ธ Androgenic alopecia (male pattern baldness) โ topical application.
- ๐ฉโ๐ฆฐ Occasionally used in female pattern hair loss (topical).
๐ Dose
- Oral: 2.5โ5 mg/day, titrated up; max 50 mg/day.
- Lower doses for elderly or dialysis patients.
- Topical: 2%โ5% solution or foam, applied to scalp BD.
๐ Interactions
- โฌ๏ธ Additive hypotension with other antihypertensives.
- Combine with loop diuretic to avoid fluid retention & ฮฒ-blocker to blunt reflex tachycardia.
โ ๏ธ Cautions
- Ischaemic heart disease (risk of angina due to tachycardia).
- Porphyria.
- Renal impairment โ may exacerbate fluid retention.
โ Contraindications
- ๐ซ Phaeochromocytoma (risk of crisis from catecholamine surge).
โ ๏ธ Side Effects
- ๐โโ๏ธ Hypertrichosis / hair growth (generalised, esp. face & arms, in systemic use).
- ๐ง Fluid retention & oedema โ always combine with a diuretic.
- โค๏ธ Reflex tachycardia, angina; risk of pericardial effusion.
- ๐คข GI upset.
- ๐ง Gynecomastia, breast tenderness (rare).
