Nitrofurantoin
โ ๏ธ Nitrofurantoin can rarely cause a lupus-like syndrome.

- ๐ First-line option for uncomplicated urinary tract infection (UTI) if renal function is normal.
- ๐ฉบ Only effective in the urinary tract (concentrates in urine, not systemic tissues).
โ๏ธ Action
- โ๏ธ Low dose โ bacteriostatic; high dose โ bactericidal.
- ๐งช Antibacterial effect confined to urine.
- ๐ก๏ธ More effective in acidic urine (pH ~5.5).
๐ Indications
- ๐ฆ Active against many Gram-positive and Gram-negative urinary pathogens.
- ๐ Treatment and prophylaxis of recurrent UTI.
๐ Dose
- ๐ 50โ100 mg QDS, or 100 mg MR BD (modified-release also suitable).
- ๐ Usual course: 3 days (women), up to 7 days if complicated infection, in men, or in pregnancy.

๐ Interactions
- โ ๏ธ May precipitate haemolysis in patients with G6PD deficiency.
โ ๏ธ Cautions
- โ Do not use if eGFR < 45 ml/minute (low urinary levels + โ toxicity).
- ๐คฐ Avoid in late pregnancy (risk of neonatal haemolysis).
- ๐ซ Use cautiously in chronic lung disease (can cause pulmonary fibrosis).
- ๐ Long-term use requires monitoring for pulmonary, hepatic, and neurological toxicity.
โ Contraindications
- eGFR < 45 ml/minute.
- History of hypersensitivity to nitrofurantoin.
๐ฅ Side Effects
- ๐คข GI: anorexia, nausea, vomiting.
- ๐งช Hepatic: hepatitis, cholestatic jaundice, rare pancreatitis.
- ๐ซ Respiratory: pulmonary infiltrates, interstitial fibrosis (chronic use).
- ๐ฉธ Haematological: haemolytic anaemia (esp. G6PD deficiency).
- ๐ง Neurological: peripheral neuropathy (with prolonged therapy).
- ๐ง Brown urine (benign but alarming to patients).
- ๐งช Can cause false-positive urine glucose tests.
