𧬠Aetiology
- Niacin is required for synthesis of NAD and NADP, essential cofactors in oxidative metabolism.
- Critical for energy production in all living cells.
- Some hepatic storage exists but limited.
- π Pharmacological niacin lowers LDL & VLDL cholesterol.
π Types
- π₯ Primary pellagra: Dietary deficiency of niacin/tryptophan.
- π§ͺ Secondary pellagra: Due to impaired absorption, metabolism, or increased demand (alcoholism, isoniazid therapy, chronic illness).
π Pearl: Pellagra is a recognised complication of isoniazid therapy, which blocks tryptophan β niacin conversion.

- π©Έ Mucosal involvement: vaginitis, oesophagitis
- π Dermatitis: photosensitive rash, Casalβs necklace around the neck (pathognomonic)
- π§ Neuro: irritability β depression β progressive dementia
- β οΈ Untreated disease progresses to death
π Investigations
- π©Έ Anaemia (often microcytic hypochromic)
- β¬οΈ Hyperhomocysteinaemia (due to impaired NAD metabolism)
- π Low serum niacin levels (confirmatory)
π Management
- π Oral nicotinamide supplementation (dramatic clinical improvement in daysβweeks)
- π₯ Optimise overall nutrition: protein, vitamins, and minerals
- π½ Public health: maize flour fortification with nicotinamide for long-term prevention
