- ๐ Broad-spectrum penicillin antibiotic.
- ๐ฆ Belongs to the beta-lactam class, related to amoxicillin.
- โ ๏ธ Less commonly used now due to resistance, but still important for some indications (e.g. Listeria).
โก Mechanism of Action
- Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs).
- Blocks final transpeptidation step โ cell wall weakening and lysis.
- Bactericidal, time-dependent killing.
๐งฌ Activity
- Some Escherichia coli strains.
- Proteus mirabilis, Shigella, Salmonella.
- Haemophilus influenzae (nonโฮฒ-lactamase producers).
- Some Enterococci.
- โ ๏ธ High rates of resistance among Gram-negative organisms in UK practice.
๐ฉบ Indications
- ๐ฌ๏ธ Exacerbation of COPD.
- ๐ฆ Invasive salmonellosis.
- โค๏ธ Endocarditis (often combined with gentamicin).
- ๐ง Listeria meningitis (drug of choice, often with gentamicin).
- ๐ง Urinary tract infections.
- ๐ ENT infections โ otitis media, sinusitis (though amoxicillin often preferred).

๐ Interactions
- โฌ๏ธ Probenecid increases ampicillin levels (reduced renal clearance).
- โฌ๏ธ Risk of methotrexate toxicity (displacement & โ renal clearance).
- โฌ๏ธ May reduce efficacy of the oral contraceptive pill.
โ ๏ธ Cautions
- ๐ก๏ธ Causes characteristic rash in patients with EBV/CMV (infectious mononucleosis, glandular fever).
- ๐ High risk of rash in patients with ALL/CLL.
- ๐ง Dose adjustment in renal impairment (renally cleared drug).
๐ซ Contraindications
- โ ๏ธ Penicillin allergy (risk of anaphylaxis).
- Infectious mononucleosis (rash almost universal if given).
- History of severe cholestatic jaundice/hepatic dysfunction associated with ampicillin.
๐ฅ Side Effects
- ๐ก๏ธ Maculopapular rash (esp. in EBV/CMV, ALL/CLL).
- ๐คข GI upset: nausea, vomiting, diarrhoea.
- ๐คง Hypersensitivity reactions: urticaria, anaphylaxis (rare but serious).
- ๐ฉธ Rare: haemolytic anaemia, thrombocytopenia, eosinophilia.
- ๐ฆ Candida overgrowth (oral/vaginal thrush) with prolonged use.
