- ๐ Bisphosphonate used in postmenopausal osteoporosis, Pagetโs disease, bone metastases, and breast cancer with bone involvement.
โก Mechanism of Action
- โฌ๏ธ Inhibits osteoclastic bone resorption, preserving bone density and reducing fracture risk.
๐ฉบ Indications & Administration
- Osteoporosis (postmenopausal, glucocorticoid-induced, male osteoporosis).
- Pagetโs disease of bone.
- Bone pain due to metastases (specialist use).
- ๐ Administration rules:
- Take on an empty stomach, at least 30 minutes before breakfast or other medicines.
- ๐ง Swallow whole with a full glass of water.
- ๐งโโ๏ธ Stay upright (sitting or standing) for โฅ30 minutes afterwards to prevent oesophageal irritation.

โ ๏ธ Interactions
- โฌ๏ธ Risk of GI ulcers with NSAIDs or Aspirin.
- Absorption reduced by calcium/iron supplements, antacids โ separate dosing.
๐ซ Contraindications
- Achalasia, oesophageal stricture, or motility disorders.
- Other oesophageal abnormalities delaying gastric emptying.
- โ Hypocalcaemia.
- Renal impairment: avoid if eGFR < 35 mL/min/1.73mยฒ.
๐ฅ Side Effects
- ๐คข GI upset: abdominal pain, dyspepsia, constipation, diarrhoea, flatulence.
- ๐ฉธ Oesophageal ulcer, melaena, dysphagia, distension, acid regurgitation.
- ๐ช Musculoskeletal pain, headache.
- โฌ๏ธ Hypocalcaemia and hypophosphataemia.
- ๐ก๏ธ Rash, hypersensitivity reactions.
- ๐ฆด Femoral stress fractures (rare).
- ๐ฆท Osteonecrosis of the jaw (esp. after dental extraction, cancer patients).
