The Gold score is a simple 1-question tool used to assess awareness of hypoglycaemia in adults with type 1 diabetes. NICE recommends using the Gold score or Clarke score to quantify hypoglycaemia awareness at review.
“Do you know when your hypos are commencing?”
| Score | Meaning |
|---|---|
| 1 | Always aware of hypoglycaemia starting |
| 2 | Usually aware |
| 3 | Borderline / uncertain awareness |
| 4 | Reduced awareness begins to become clinically significant |
| 5 | Often unaware |
| 6 | Usually unaware |
| 7 | Never aware of hypoglycaemia starting |
| Gold score | Interpretation |
|---|---|
| 1–2 | Normal hypoglycaemia awareness |
| 3 | Borderline awareness |
| ≥4 | Impaired awareness of hypoglycaemia |
| Autonomic / adrenergic symptoms | Neuroglycopenic symptoms |
|---|---|
| Sweating, tremor, palpitations, hunger, anxiety, tingling. | Confusion, drowsiness, irritability, slurred speech, poor concentration, visual disturbance, seizure or reduced consciousness. |
✅ Key message: A Gold score of 4 or more identifies impaired hypoglycaemia awareness and should prompt review of insulin therapy, education, technology options and safety advice.
