Metronidazole (Flagyl)
๐ Must be given orally for pseudomembranous colitis with Clostridioides difficile infection, as it acts within the bowel lumen.

- ๐ฌ Reduced within anaerobic cells to toxic metabolites that disrupt DNA and bacterial metabolism.
๐ Activity

Anaerobes including Bacteroides fragilis.
- โ
Protozoa: Trichomonas vaginalis, Entamoeba histolytica, Giardia lamblia.

Clostridium tetani and Clostridioides difficile.

๐ Interactions
- ๐ท Disulfiram-like reaction with alcohol โ flushing, abdominal pain, hypotension.
- โ ๏ธ Inhibits warfarin metabolism โ โ INR.
- ๐ Can increase lithium and phenytoin toxicity.
- ๐ซ Avoid in porphyria.
โ ๏ธ Cautions
- ๐ง Prolonged use (>4 weeks) may cause peripheral neuropathy.
- ๐คฐ Use with caution in pregnancy and breastfeeding (seek specialist advice).
- ๐ซ Porphyria.
โ Contraindications
- ๐ Specialist advice required in pregnancy, breastfeeding, and porphyria.
๐ฅ Side Effects
- ๐คข GI: nausea, vomiting, diarrhoea, metallic taste.
- ๐งช Hepatic: hepatitis (rare).
- ๐ง CNS: headache, seizures (at high doses).
- ๐ฆถ Sensory neuropathy with prolonged treatment.
