๐ฆต The commonest problem with this agent is ankle oedema, which may necessitate changing therapy, as oedema responds poorly to leg elevation or diuretics.
- Acts mainly peripherally on vascular smooth muscle, less on the heart.
โ๏ธ Mechanism
- ๐น Dihydropyridine class of calcium channel blocker (CCB).
- ๐ฉธ Vasodilator effect on vascular smooth muscle.
- ๐ Claimed to have no significant negative inotropic effect.
๐ฏ Indications / Dose
- ๐ Hypertension, Angina, Vasospasm: 5โ10 mg PO once daily (ON).
๐ Interactions
- โ ๏ธ Additive effect with other antihypertensives (risk of hypotension).
- ๐ With statins: reduce simvastatin from 40 mg โ 20 mg due to interaction (use atorvastatin if concerned).
โ Contraindications
- ๐ Hypotension.
- ๐ซ Significant aortic stenosis.
- ๐งฌ Acute porphyria.
โ ๏ธ Side Effects
- ๐ฉ Constipation, fatigue.
- โค๏ธ Reflex tachycardia.
- ๐ณ Facial flushing, headache.
- ๐ฆต Ankle oedema (common), leg ulcers (rare).
