The Abbreviated Mental Test Score (AMTS) is a quick 10-point bedside screening tool, introduced by Hodkinson in 1972, to assess global cognitive function in older adults. It samples key cognitive domains including orientation, short-term memory, attention and general knowledge. It is widely used in UK hospitals, GP surgeries and care homes to screen for delirium, dementia, and other causes of cognitive impairment.
| Domain | AMTS Components | Clinical Significance |
|---|---|---|
| Orientation | Age, year, place, current monarch | Often impaired early in delirium; temporal disorientation common in dementia |
| Attention | Counting backwards 20 → 1 | Reduced attention is a core feature of delirium |
| Short-term Memory | Recall of address | Early hippocampal dysfunction in Alzheimer’s disease |
| Remote Memory / General Knowledge | WWI start date | Usually preserved until later dementia stages |
| Personal Semantic Memory | Date of birth | Usually intact unless significant impairment |
An AMTS ≤6 suggests significant impairment, but interpretation must consider:
| Tool | Time | Strengths | Limitations |
|---|---|---|---|
| AMTS | 2–3 mins | Very quick; easy in acute settings | Limited executive/visuospatial testing |
| 4AT | 2 mins | Better delirium detection; includes alertness & attention | Less detailed for chronic impairment |
| MMSE | 7–10 mins | Broader coverage | Copyright restrictions; educational bias |
| MoCA | 10–15 mins | More sensitive for mild cognitive impairment; executive function tested | Longer to administer |
The AMTS is a rapid bedside screening tool sampling orientation, memory and attention. It is particularly valuable in acute UK settings to identify possible delirium or established cognitive impairment. A low or deteriorating score should trigger urgent assessment for reversible causes and more detailed cognitive testing. Interpret results within full clinical context, including baseline function and collateral history.
