Minocycline

- ๐ Tetracycline-class antibiotic.
- ๐ Long half-life allows once or twice daily dosing.
โ๏ธ Action
- ๐ Inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit.
- ๐ Bacteriostatic effect.
๐ Indications
- ๐ Acne vulgaris (often prolonged courses >3 months).
- ๐งซ Historically used for meningococcal prophylaxis, but largely abandoned due to vestibular side effects (dizziness, vertigo).
๐ Interactions
- โ ๏ธ Avoid oral retinoids (e.g., isotretinoin, acitretin) โ risk of idiopathic intracranial hypertension (pseudotumour cerebri).
- โฌ๏ธ May reduce efficacy of oral contraceptives (use additional contraception).
- โ๏ธ Strontium ranelate reduces absorption of minocycline.
- ๐ฉธ Can โ INR in patients taking warfarin.

May be used in renal failure (unlike older tetracyclines).
- ๐ Photosensitivity โ advise sunscreen and avoidance of excessive sunlight/UV lamps.

- ๐ Acne vulgaris: 100 mg PO OD or BD (often long-term).
๐ฅ Side Effects
- ๐คข GI: nausea, vomiting, diarrhoea, antibiotic-associated colitis.
- ๐ก๏ธ Dermatological: rash, dermatitis, StevensโJohnson syndrome.
- ๐ง Neurological: idiopathic intracranial hypertension.
- ๐งช Hepatic: hepatotoxicity, pancreatitis (โ risk with prolonged use โ monitor LFTs).
- ๐จ Pigmentation of skin, nails, or mucosa with prolonged courses.
- ๐ Alopecia (rare).
- ๐ก๏ธ Drug-induced lupus erythematosus (with prolonged use).
