Most of these are due to small lacunar type strokes affecting the brainstem

| Weber syndrome | Oculomotor palsy with contralateral hemiparesis and contralateral UMN VII palsy. Infarct Base of midbrain involves IIIrd nerve and corticospinal tracts |
| Claude syndrome | Oculomotor palsy with contralateral cerebellar ataxia, contralateral hemiparesis and tremor. Infarct to cerebral peduncle and involves red nucleus |
| Benedikt syndrome | Oculomotor palsy with contralateral cerebellar ataxia and tremor and corticospinal signs. Stroke affects the III red nucleus and corticospinal tract |
| Parinaud syndrome | Paralysis of upward gaze and fixed pupils with convergence disorders and pupillary and lid disorders. Infarct around the periaqueductal structures and Dorsal midbrain |
| Nothnagel syndrome | Ipsilateral IIIrd nerve lesions and ipsilateral cerebellar ataxia. Stroke affects Tectum of midbrain takes out III and superior cerebellar peduncle |
| Millard Gubler syndrome | Ipsilateral LMN Facial weakness and abducens weakness and a contralateral hemiplegia. Infarct at the pontomedullary junction takes out VII nucleus and VI nerve and corticospinal tract |
| Wallenberg syndrome | Ipsilateral V,IX,X,XI palsy. Horner syndrome. Cerebellar ataxia and contralateral pain and temperature sense impaired = Wallenberg syndrome. Lateral spinothalamic tract, descending pupil dilator fibres and spinocerebellar and olivocerebellar tracts as well as V,IX,X,XI nuclei and lateral tegmentum of medulla (Lateral medullary syndrome) |
