Oxygen treats hypoxaemia, not breathlessness alone; excess inspired oxygen may aggravate hypercapnia in susceptible patients and mask worsening ventilation.
Check respiratory rate, work of breathing, pulse oximetry, oxygen delivery device and whether the patient is at risk of type 2 respiratory failure.
For most acutely ill adults prescribe a target SpOโ of 94โ98%; for patients at risk of hypercapnic respiratory failure generally use 88โ92% pending blood gas review and an individualised plan. Emergencies may require immediate oxygen before the prescription is written.
Record oxygen device, flow/concentration, target range and reassessment. Obtain blood gas analysis when hypercapnia or acidosis is suspected, and treat the underlying problem.
A normal SpOโ does not rule out hypercapnia, severe anaemia or carbon monoxide poisoning. Oxygen requirement is a clinical deterioration sign, not a diagnosis.
UK reference: BTS oxygen guideline. Focused editorial source check, 7 November 2026; this is not independent clinical or pharmacy sign-off. Verify current local policy, BNF/BNFC and specialist advice before treatment. Not a prescribing protocol.
