Antibiotics: 48-hour review โ ๏ธ ๐
Educational on-call framework โ not a treatment protocol. Reassess antibiotics at 48โ72 hours using examination, cultures, clinical progression and a documented plan.
Clinical reassessment
- Is bacterial infection still likely? Review vitals, source, cultures, imaging and response.
- Has source control (drainage, device removal or surgery) been addressed?
Change the prescription
- Stop if no bacterial indication; narrow spectrum once microbiology available; move IV to oral when stable, absorbing and suitable.
- Document the indication, drug, route, duration/stop date and next review; check allergies, renal function and therapeutic drug monitoring.
Important exceptions
- An unstable patient requires immediate reassessment and escalation; follow suspected sepsis rather than waiting for the review date.
- For time-critical infection treatment or a narrow therapeutic index, involve microbiology/pharmacy, especially gentamicin or vancomycin.
