IV fluids: prescribing and reassessment ⚠️ 💧

Safe intravenous fluid prescribing begins with the clinical indication: resuscitation, maintenance, replacement of losses or redistribution.
Before prescribing
- Assess haemodynamics, JVP, oedema, respiratory signs, intake/output, weight, renal function, sodium/potassium and ongoing losses.
- Identify heart failure, renal failure, frailty and high risk of fluid overload; oral/enteral fluids may be preferable.
Monitor and review
- Specify fluid type, rate/volume, indication, monitoring and review time using local policy/NICE guidance; avoid automatic repeat bags.
- In shock, give the relevant emergency resuscitation bolus and reassess urgently; isolated maintenance prescribing is not adequate resuscitation.
- Review fluid balance, daily electrolytes where indicated and ongoing need; seek senior advice for complex sodium disorders.
Clinical reasoning: Fluid therapy is a drug prescription with potential adverse effects; sodium disorders and pulmonary oedema often result from mismatched fluid choice or insufficient reassessment.
