Premature ovarian insufficiency 🩺

Premature ovarian insufficiency (POI) means loss of normal ovarian function before age 40, presenting with oligo/amenorrhoea and often menopausal symptoms or infertility.

Assessment

Exclude pregnancy, assess thyroid/prolactin and appropriately timed FSH/oestradiol using current specialist criteria; do not diagnose from one unspecific result alone.
Ask about chemotherapy, ovarian surgery, autoimmune disease and family history; offer indicated genetic testing and counselling.

Management

Discuss hormone replacement until the usual age of menopause in eligible patients, bone and cardiovascular health, contraception, fertility and psychological support.
Refer to gynaecology/endocrinology or fertility specialists; intermittent ovulation can still occur.
