Respiratory deterioration during or soon after transfusion may reflect fluid overload (TACO), inflammatory lung injury (TRALI), anaphylaxis, haemolysis or bacterial contamination.
Look for hypoxia, tachypnoea, new crackles or bilateral infiltrates. High blood pressure, raised JVP and fluid overload suggest TACO but cannot alone establish the diagnosis.
Stop the transfusion, maintain IV access per local reaction policy, assess ABCDE, give oxygen as needed and summon urgent clinical/transfusion-laboratory support.
Recheck identification and compatibility documentation; organise observations, chest imaging and other tests in discussion with haematology. Return the component and report the reaction per local policy.
Do not automatically give diuretics for all transfusion-associated respiratory distress: TRALI is not a fluid overload syndrome. Frailty, heart failure and renal impairment raise TACO risk.
UK reference: Dorset County Hospital transfusion protocol. Focused editorial source check, 7 November 2026; this is not independent clinical or pharmacy sign-off. Verify current local policy, BNF/BNFC and specialist advice before treatment. Not a prescribing protocol.
