A misplaced feeding tube can deliver feed or medicine into the respiratory tract and cause fatal aspiration or lung injury.
Before use, confirm documented insertion and the trust-approved gastric placement assessment, including patient identity, tube type and external length.
Do not feed, flush or administer medicines until first placement is confirmed by an acceptable method: gastric aspirate pH 1โ5.5 with approved strips or a correctly interpreted current X-ray following local policy.
If pH is non-confirmatory, aspirate unavailable or radiography unclear, escalate to a trained senior clinician; ensure radiographic checks include course and tip positioning and are documented.
Never use the whoosh test, blue litmus or absence of cough as proof of gastric placement. Seek re-confirmation after displacement or new respiratory symptoms as required locally.
