Pregnancy of unknown location โ safety-netting ๐จ ๐ฉบ
๐ง Clinical mechanism: A positive pregnancy test with no visible intrauterine or extrauterine pregnancy on scan may represent a very early pregnancy, failing pregnancy or ectopic pregnancy.
๐ Recognise
- Positive test, pain or bleeding with nondiagnostic ultrasound; an initially low hCG does not exclude tubal rupture.
๐ First assessment and action
- Arrange early pregnancy/gynaecology review; if unstable use emergency haemorrhage pathway. Provide explicit 24-hour return instructions.
๐งช Investigations and reassessment
- Transvaginal ultrasound and serial serum hCG near 48 hours apart, interpreted with symptoms and specialist review.
โ ๏ธ Pitfall / escalation
- Serum hCG cannot locate a pregnancy; worsening pain or syncope overrides a reassuring numerical trend.
