Glycogen stores, feeding transition and maternal diabetes can alter neonatal glucose adaptation; symptomatic or persistent low glucose risks neurological injury.
Screen babies with locally defined risk factors; assess for jitteriness, poor feeding, hypotonia, apnoea, hypothermia or seizures. Glucose meters may be less accurate at low values.
Urgently involve the neonatal team for symptomatic, severe or persistent hypoglycaemia. Follow the neonatal glucose/feeding protocol, which may require supervised feeding, dextrose gel or intravenous glucose.
Confirm measurements with the appropriate laboratory or neonatal method where indicated, and reassess glucose at the specified interval. Persistent episodes warrant consideration of infection or metabolic disease.
Do not apply adult hypoglycaemia thresholds, adult concentrated dextrose formulations or a paediatric resuscitation recipe without neonatal guidance.
