Child status: buccal midazolam ๐จ ๐ถ
๐จ ๐ถ PAEDIATRIC HIGH ALERT โ EDUCATIONAL DRAFT, NOT A PRESCRIBING ORDER. Drug choice, indication, formulation, dose, route and monitoring require current BNF/BNFC, product SmPC and local NHS protocol. Source reviewed 3 November 2026; independent clinical/pharmacy sign-off pending.
๐งฌ Mechanism and reason for use
- Midazolam enhances GABA-A signalling; buccal rescue delivery is useful before IV access is established.
- Community/ward emergency prolonged convulsive seizure where individualised plan or local paediatric status guideline authorises buccal administration.
๐ Indication-specific UK regimen / dosing boundary
- RCUK 2025 algorithm: buccal midazolam 0.3 mg/kg (maximum 10 mg) in the paediatric convulsive status pathway, but RCUK also directs users to BNFC for exact age-related dosing and product guidance.
- Age-banded licensed buccal formulations vary: check BNFC, brand/device, the individual rescue plan and prior benzodiazepines before administration.
๐ Before writing a prescription
- Confirm route, concentration in mg/mL, weight/age, rescue plan, number/time of doses already given; summon paediatric help and check glucose.
๐งช Monitoring and reassessment
- Airway/breathing, conscious level, blood pressure, persistent/recurrent seizure; ready for ventilation.
๐จ Serious pitfalls / when to escalate
- Confusing concentration with volume may cause overdose. Two benzodiazepine doses total (including prehospital treatment) is a key safety check.
