Time-critical inpatient drugs โ ๏ธ ๐
Educational on-call framework โ not a treatment protocol. Medication reconciliation means understanding the actual home regimen, not merely copying electronic repeat prescriptions.
On admission
- Ask about dose timings, last dose, medication source, allergies, swallowing/enteral route, and pharmacist-verified reconciliation.
- Flag insulin, Parkinson medicines, antiepileptics, anticoagulants, steroids and critical replacement therapies on the ward chart.
If NBM or unable to swallow
- For Parkinson treatment, arrange urgent specialist alternative and avoid abrupt interruption; see co-careldopa.
- For insulin and diabetes, protect basal insulin according to the individual plan and check hypoglycaemia/DKA risk; see rapid insulin and basal insulin.
- Do not crush modified-release or enteric-coated preparations without pharmacy advice.
When a dose is missed
- Check the drug-specific missed-dose guidance and time interval; do not automatically double the next dose.
- Escalate early for seizures, Parkinson deterioration, steroid dependence or other time-critical therapy.
