Child antibiotics: prescribe safely โ ๏ธ ๐ถ ๐
๐จ ๐ถ PAEDIATRIC HIGH ALERT โ EDUCATIONAL DRAFT, NOT A PRESCRIBING ORDER. Drug choice, indication, formulation, dose, route and monitoring require current BNF/BNFC, product SmPC and local NHS protocol. Source reviewed 3 November 2026; independent clinical/pharmacy sign-off pending.
๐งฌ Mechanism and reason for use
- Paediatric pharmacokinetics depend on age and maturation of renal/hepatic clearance; antibiotic dose and interval depend on weight, age, infection site and organism.
- Suspected severe infection where antibiotics must not be delayed, with urgent paediatric senior/microbiology involvement.
๐ Indication-specific UK regimen / dosing boundary
- No universal paediatric antibiotic doses; obtain current BNFC/local paediatric antimicrobial protocol for exact mg/kg per dose, maximum adult dose, interval, route, and course.
- For neonates and meningitis/sepsis, use age-specific emergency protocols; do not scale adult ceftriaxone, gentamicin or benzylpenicillin by intuition.
๐ Before writing a prescription
- Confirm measured weight (kg), exact age/gestation, allergy type, prior antibiotic, indication, renal function, culture sampling and therapeutic drug monitoring if indicated.
๐งช Monitoring and reassessment
- Clinical response, temperature/perfusion, renal function, antibiotic level monitoring where required, and dose adjustments after cultures.
๐จ Serious pitfalls / when to escalate
- mg/kg per dose versus mg/kg per day, wrong maximum and confused mg/mL concentration are common serious errors.
