🧬 Pharmacology and role

Trimethoprim plus sulfamethoxazole; specialist antimicrobial with important haematologic and metabolic interactions.

🔎 Before giving

Check kidney function, potassium, full blood count and the indication; some regimens are weight-based and substantially higher than routine antibacterial doses.
Avoid co-prescribing methotrexate unless a specialist has managed the substantial marrow-suppression risk; also check warfarin, ACEi/ARB and spironolactone.
Review sulfonamide allergy and whether PJP treatment versus prophylaxis is intended.

🚨 Avoidable error / escalation

Bone marrow suppression, hyperkalaemia and serious rashes require prompt recognition; do not substitute routine UTI dosing for Pneumocystis treatment.
