Unconjugated bilirubin can cross the immature bloodโbrain barrier at high levels, while prolonged conjugated jaundice may point to biliary obstruction or liver disease.
Jaundice in the first 24 hours, poor feeding, lethargy, abnormal tone, pale stools or dark urine are red flags. Interpret bilirubin in relation to gestational age and exact postnatal hours.
For visible jaundice during the first 24 hours, arrange an urgent serum bilirubin measurement and neonatal review. Escalate a neurologically unwell infant immediately.
Use age-in-hours treatment charts and the correct serum/transcutaneous measurement pathway; investigate haemolysis, infection and prolonged jaundice with neonatal input.
Do not estimate bilirubin numerically from skin colour or use a single fixed threshold at all ages. A well-appearing baby with prolonged jaundice still needs assessment for conjugated hyperbilirubinaemia.
UK reference: NICE CG98 neonatal jaundice. Focused editorial source check, 7 November 2026; this is not independent clinical or pharmacy sign-off. Verify current local policy, BNF/BNFC and specialist advice before treatment. Not a prescribing protocol.
