First assessment
- Ask the patient/carer about their exact home dosing times; reconcile modified-release formulations and any infusions or pumps with pharmacy.
- Missed levodopa can rapidly worsen rigidity, swallowing, mobility and consciousness and, in severe cases, cause acute akinesia or a neuroleptic-malignant-like syndrome.
If the patient is nil by mouth
- Check swallowing and absorption; urgently contact the Parkinson team/pharmacist for a validated alternative preparation or route.
- Avoid abrupt withdrawal and seek review of dopamine-blocking medicines such as metoclopramide and haloperidol where possible.
- Document timed administration and a named review plan; do not improvise conversion doses.
